Job Description

POSITION SUMMARY
Responsible for counseling, referrals, clinical assessments, development and implementation of individual treatment plans, safety plans, and case management/care coordination for identified students. Works collaboratively with others to promote the wellbeing of students.
ESSENTIAL DUTIES, FUNCTIONS, & RESPONSIBILITIES
Attends IEP’s, FCS’s, and other required student meetings.
Attends required program meetings including weekly individual and group sessions.
Utilizes electronic record keeping system and inputs clinical records for counseling services in a timely manner.
Completes intake assessment, progress notes, treatment plans and discharge summaries in accordance with timelines set in department policies and procedures.
Submits a minimum of 15 billable contact hours per week, with an average of 20 billable hours per week.
Will facilitate 2-3 groups per week. Caseloads will be adjusted to achieve average billable contact hours, but will not exceed 30 students. 
Provides mental health counseling to students utilizing a short-term therapy model. 
Provides individual, family, and group therapy regarding emotional, psychological, substance abuse, behavioral, and/or relationship/family issues.
Coordinates with parents/guardians, staff members, administrators, and others to develop treatment plans and/or individualized intervention plans for students, when applicable.
Provides crisis intervention to students during usual work hours.
Carries the rotating crisis phone 24 hours/7 days to provide phone coaching for school staff when a student is in crisis.
Determines the effectiveness and progress of individualized treatment plans and/or individualized intervention plans and makes recommendations if modification of services or outside referral is necessary. Conducts regular outcomes assessments to determine effectiveness of treatment.
Guides students in the development of skills and strategies to address individual needs and challenges.
Maintains confidential case files as required by the HIPAA Privacy Act.
Ensures release of information forms are obtained and completed, if necessary.
Coordinates care with necessary parties for the purpose of meeting a student’s need ensuring student confidentiality and HIPAA are not violated.
Assists in the training and coaching of staff in mental and behavioral health issues as well as crisis intervention skills.
Conducts home visits and/or communicates with student’s family regarding continuity of services to ensure optimum student progress, when applicable.
Participates in program outreach during school activities and during the academic day (career fair, health class etc.).
Reviews and responds to e‐mails, voicemails, and other forms of communication within one business day.
May participate in special programs and/or events.
Must be able to work flexible weekend and evening hours.
KNOWLEDGE, SKILLS, & ABILITIES
Knowledge of principles, methods, assessments, standards, procedures of diagnosis and treatment plans as well as counseling interventions. 
Ability to apply counseling methods and techniques for adolescents (age 12 and older).
Knowledge of case management, community resources, legal and ethical issues related to student’s rights and parent advocacy. 
Ability to establish positive working relationships with students’, staff, parents, and other professionals. 
Ability to plan and organize, meet schedules and deadlines for each job component. 
Ability to maintain files and records and be in compliance with the HIPAA Privacy Act.
Ability to communicate effectively both verbally and in writing with students, family, staff, and community agencies.
Flexibility, organization, decision-making and problem-solving skills.
Interpersonal skills with work with diverse populations both in person and on the telephone.  
Ability to collaborate with school administration, faculty, staff, students, parents, and community providers to achieve optimum health outcomes for the SFIS School Community.
Ability to establish and maintain respectful working relationships with students, parents, and staff.
Ability to prepare accurate, complete, and legible reports presenting detailed and accurate information.
QUALIFICATIONS
Minimum Qualifications
Must have LHMC, LPCC, LMSW, LCSW, or equivalent, license.  
One (1) year of experience in counseling, preferably with Native American students.
Must maintain required continuing education (CEs) and other requirements necessary to maintain professional licensure.
Preferred Qualifications
Experience working with Native American students and Native American or Tribal entities, preferred.
OTHER REQUIREMENTS
Must possess a valid NM driver’s license and must complete and successfully pass a background investigation, including FBI fingerprint check.
PHYSICAL DEMANDS
While performing the duties of this position; sitting, standing, lifting, carrying (up to 30 pounds), reaching, squatting, kneeling, and moving light furniture may be required. Must be able to work within various degrees of noise, temperature, and air quality. Interruptions of work are routine. Flexibility and patience are required. Must be self-motivated and able to complete job assignments without direct supervision. Have manual and finger dexterity. Occasionally required to reach above shoulder level, repetitive hand, and arm motions; must be able to see, read, and record small numbers.
WORK ENVIRONMENT, SAFETY & EQUIPMENT
Work is generally performed in an office setting. Frequent use of communication equipment and computers.  Evening, holiday, and weekend work may be required. Must be able to work within various degrees of noise, temperature, and air quality. Interruptions of work are routine. Flexibility and patience are required. Must be self-motivated and able to complete job assignments without direct supervision. May require traveling to various communities to meet with students, parents, guardians, Tribal social services, and other service providers.
Safety and Health Requirements:
Proper posture is required to eliminate acute and chronic low back pain.
Complete all required safety and health training.
Knowledge of universal hygiene precautions and knowledge of safe school precautions.
The employee must adhere to SFIS safety protocols and crisis plans.
Equipment/Material Handled:
Must know how to properly operate, or be willing to learn to operate, all multi-media equipment, software, and current technology.
CONDITIONS
Any employee may be required to stay at or return to work during incidents and/or emergencies to perform duties specific to this classification or to perform other duties as requested in an assigned response position. This may require working a non-traditional work schedule or working outside normal assigned duties during the incident and/or emergency.
